Key Responsibilities:
• Deliver client services tailored to individual needs
• Facilitate home visits and wellness plan development
• Record services in the Online Health and Access Tracking System
• Organize and coordinate educational activities
• Maintain confidentiality and adhere to privacy regulations
Requirements:
• Degree or diploma in social services
• 1+ years in client services or non-profit sector
• Experience working with children aged 0-18
• Proficiency in Microsoft Outlook, Excel, Word, and PowerPoint
• Ability to lift 11.5kg when necessary
