Drive health equity as a Provincial Hep C Program Coordinator with PASAN. This role supports incarcerated individuals at risk of HIV/Hep C through advocacy, education, and comprehensive care.
At PASAN, we're focused on improving health outcomes for those impacted by incarceration. As the Coordinator, you will manage in-reach programming, advocate for prisoners living with Hep C, and promote access to testing and treatment. This role requires excellent communication skills, a commitment to harm reduction principles, and an understanding of the social impacts of the criminal justice system.
Key Responsibilities:
• Coordinate prison in-reach programs around STBBI prevention
• Provide case management and advocacy for Hep C patients
• Collaborate with community stakeholders to enhance support
• Develop and maintain relationships with local health services
• Monitor and evaluate program effectiveness and delivery
Requirements:
• Experience in HIV and Hep C education and resources
• Solid understanding of harm reduction practices
• Excellent interpersonal skills with ability to collaborate
• Proficiency in Microsoft Office tools
• Valid class G Driver’s License and clearance for prisons
