Seeking a Community Care Navigator for the Royal Free London NHS Foundation Trust, focusing on integrating care for residents with long-term health conditions. This role emphasizes team-oriented support in the Enfield area.

As part of the Enfield Integrated Proactive Care Service MDT, you will facilitate access to essential health and social care services. Your work will aim to improve individual wellbeing and facilitate independence within the community. Engage with GPs, local authorities, and voluntary sectors to ensure comprehensive, user-centered care.

Key Responsibilities:
• Process referrals and assess needs from multiple sources
• Identify and connect unpaid carers to resources
• Develop support plans to enhance care continuity




• Liaise effectively among healthcare professionals
• Assist individuals in navigating available services

Requirements:
• Background in healthcare or community services
• Excellent interpersonal and organizational skills
• Knowledge of health and social resources in Enfield
• Capacity to promote patient empowerment
• Experience in care coordination is a plus
