Job Summary
The Community Disability Practitioner (CDP) provides direct support to Individuals Accessing Services (IASs) in achieving their goals and leading fulfilling lives. This is a senior frontline position that supports adults or youth with more complex developmental and physical disabilities. This includes providing medical, technological, environmental, emotional, physical, and behavioural support to IASs. The CDP helps individuals reach their goals by actively engaging with them, learning their interests, and supporting them to succeed.

We are seeking a skilled practitioner with experience supporting adults with developmental disabilities and complex needs, including those who may be justice‑involved, unhoused, or in unstable housing. You bring strong knowledge of the justice system, community housing options, and local resources, and you use a harm reduction, person‑centered approach to help people safely access services, stabilize in the community, and work toward their own goals, even when situations are chaotic, high‑risk, or emotionally charged.

Accountablilities

1. Ensure that the day-to-day needs of IASs with more complex needs are met, and that Vantage policies and procedures are being consistently followed

- Proactively build positive relationships with IASs
- Provide medical, technological, environmental, emotional, physical and behavioural support to IASs
- Advocate for IASs in the community and ensure that IASs are being supported and actively integrated in the community
- Ensure to safeguard IASs’ rights/values and empower them to make their own decisions
- Be organized and proactive in the scheduling of IAS’ daily activities
- Administer medication correctly for each IAS and follow appropriate policies and procedures
- Submit verbal and written reports of any accidents, incidents, or illnesses to the Team Leader immediately and guardian and worker as required
- Assist in coordinating or providing transportation for IAS needs
- Ensure that IASs are taken to medical appointments if needed
- Ensure that all health and safety protocols are followed consistently
- Ensure that program administrative requirements are consistently and effectively maintained (daily journals, outcome tracking, health summaries, IAS schedules, etc.) in accordance with agency procedures
- Ensure that all IAS documentation is completed in an effective and timely manner using the 4Cs: clearly, concisely, coherently, and considerately
- Ensure all administration and program activities and requirements are met in appropriate timelines as set out by internal and external contacts

2. Participate in the development and implementation of IAS support plans and achievement of IAS goals

- Participate in the research, development, and implementation of programs to meet IAS goals
- Provide input as needed in the development and updating of Individual Support Plans (ISPs), Planned Positive Practices (PPPs) and Restrictive Procedures (RPs)
- Liaise with Team Leader to complete and update support plans and ensure that goal tracking is completed throughout the year (outcome tracking,



changes to IAS information, and all relevant information is up to date)
- Participate in IAS network and team meetings as required

3. Be a positive and contributing member of the team

- Be a positive role model and help to mentor new workers during the onboarding process
- Help create a culture of recognition and appreciation on the team
- Actively participate and contribute new ideas in team meetings
- Consistently arrive for shifts on time and notify the Peer Leader or Team Leader immediately if an unexpected situation arises that causes you to be late

4. Provide IAS with a Harm Reduction approach

- Support IAS in identifying safe options and taking steps to ensure immediate safety
- Provide crisis intervention and timely support during high-stress or emergency situations
- Connect IAS to appropriate resources and long-term supports, including housing, counseling, legal supports, and community programs

Communication: able to listen to IASs effectively and communicates in a way that builds rapport and trust with IASs, team members, and stakeholders; and writes documentation clearly, coherently, concisely, and considerately

Adaptability: adapts to changing program needs and a variety of situations that arise daily; and supports the cultivation of a work environment that can manage change well

People-focused: fosters an inclusive and positive work environment for the team’s diverse staff; and promotes cooperation and commitment to achieve goals

Learning: proactively identifies ways for self-improvement and leadership development, and fully supports the growth and development in all workers

Advocacy: provides supports and advocates for the independence, equal opportunity, and participation of IASs in society

Qualifications/ Experiences

- Diploma in Disability Services (or a relevant discipline) or a minimum of Foundations or PCA Training
- Minimum of two years of progressive direct service experience
- Experience working with Complex Needs individuals
- Experience using a harm reduction approach
- Mandatory training (First Aid & CPR, Abuse Protocol, The Mandt System, ASIST, Mental Health First Aid, Med Assist, Universal Precautions, and PPP & RP) and other individual specific training (training and recertification opportunities are provided by Vantage)
- Must have a clear Vulnerable Sector Check (applied within the last 6 months)
- Proficient digital skills (computer and writing/reporting skills and basic internet search skills)




- Flexibility to work various shift schedules and in physically demanding situations
- Possession of a reliable car with personal liability insurance, providing a minimum coverage of two million ($2,000,000.00)
- Possession of a Class 5 - Non GDL Driver’s License with a clean drivers abstract and no restrictions
