We are seeking a dedicated and organized Case Manager to join our team and help ensure that each client receives high-quality, coordinated care based on their individual needs.

Position Summary

The Case Manager will oversee the coordination and management of care for clients receiving home healthcare services. This role involves assessing client needs, developing and monitoring care plans, coordinating with caregivers and healthcare professionals, communicating with families, and ensuring that services are delivered safely and effectively.

Key Responsibilities

 Conduct client assessments and identify individual care needs.

 Develop, implement, and regularly review individualized care plans.

 Coordinate home healthcare services and ensure appropriate staffing and coverage.

 Monitor clients’ progress and respond appropriately to changes in their condition or circumstances.

 Communicate regularly with clients, families, caregivers, nurses, physicians, and other healthcare professionals.

 Conduct home visits as required to assess clients and monitor service quality.

 Address client and family concerns promptly and professionally.

 Maintain accurate, timely, and confidential client documentation.

 Ensure care is delivered in accordance with company policies, applicable regulations, and professional standards.

 Monitor caregiver performance and provide guidance and support when needed.





 Identify potential health, safety, or care concerns and escalate them appropriately.

 Participate in care coordination meetings and case reviews.

 Assist with maintaining continuity of care during changes in client needs or staffing.

Qualifications

 Bachelor’s degree in Nursing, Social Work, Healthcare Administration, Public Health, or a related field preferred.

 Previous experience in case management, care coordination, home healthcare, nursing, social services, or a related healthcare setting.

 Strong understanding of client-centered care and care planning.

 Excellent communication, organization, and interpersonal skills.

 Ability to manage multiple clients and competing priorities effectively.

 Robust documentation and record-keeping skills.

 Ability to work independently and as part of a multidisciplinary team.

 Proficiency with electronic health records and Microsoft Office or similar software.

 Valid driver’s license and reliable transportation may be required.

Preferred Skills

 Experience working with elderly, disabled, chronically ill, or medically complex clients.

 Knowledge of home healthcare regulations and best practices.

 Strong problem-solving and conflict-resolution skills.

 Compassionate, professional, and client-focused approach.

 Ability to handle sensitive and confidential information appropriately.
