Responsibilities
- Be able to communicate professionally engage with patients, families, staff and funder organizations.
- To process inquiries and handle funder referrals and updates in Alayacare software.
- Spend the time to review and collaborate with Supervisors, field employees and clients to assess service needs.
- Schedule and assign care providers based on client needs, geographic area, staff skill-set and availability.
- Must coordinate and ensure consistent homecare assignments and service delivery.
- Always notify and keep clients and staff informed of initial and ongoing schedules.
- Optimize staff schedules fully to meet their availability and client care needs.
- Always document and maintain accurate records in Alaycare.
- Recruit, onboard and train new community health staff with support from Director.
- Process documents by entering documents into Alayacare.
- Distribute and order supplies used by field staff as required.
- Try to resolve matters by addressing patient concerns promptly and then follow escalation pathways as required.
- Act as the main liaison between clients and internal teams, managing ongoing communication and ensuring client satisfaction
- Coordinate project timelines, deliverables, and resources to meet client expectations efficiently
- Skills
- Based on the Key Activities, this position requires knowledge of:
- A minimum of two years related customer service experience, preferably in healthcare.
- Strong ability to communicate effectively, both written and orally, in English.
- Excellent computer skills (Windows, Microsoft office, Procura, Alayacare software).
- Able to multi-task and works well under pressure to meet timelines for completion.
- Proficient in using smart phones applications for scheduling.
- Positive work and attendance performance essential.
- Client Care practices/procedures of Merging Homecare.
- Merging Homecare’s mission, mandate and vision.
- Knowledge of terminology used in clinical, nursing and home health/home support care.
- Strong project management skills with the ability to prioritize tasks effectively in a busy environment
- Excellent communication skills—both verbal and written—to clearly convey information and build rapport

Pay: $24.00-$28.00 per hour
