Job Title: Registered Nurse

Reports to: Executive Director

POSITION SUMMARY The Registered Nurse (RN) provides compassionate, resident-centered nursing care to individuals receiving hospice palliative care at Jessica’s House Hospice (JHH). Working as part of an interdisciplinary team, the RN is responsible for assessing, planning, implementing and evaluating care to support residents and their families throughout the end-of-life journey while promoting dignity, autonomy, comfort and quality of life.

The RN practices in accordance with the Regulated Health Professions Act, College of Nurses of Ontario (CNO) standards and the policies and procedures of JHH, while supporting the organization’s mission, vision and values.

POSITION TYPE The role of RN may be a full time, part time, casual, temporary or permanent position and is employed by JHH, the operating company. The position is in Exeter, ON.

RESPONSIBILITIES

- Assume responsibility for all aspects of resident care in accordance with the Regulated Health Professional Act and the College of Nurses Ontario (CNO) standards.
- Provide professional nursing care to residents in accordance with the nursing process and hospice philosophy of care.
- Conduct comprehensive and ongoing pain and symptom assessments and implement appropriate interventions.
- Develop, implement and evaluate individualized care plans to support resident comfort and quality of life.
- Review and update resident care plans regularly based on resident needs and changes in condition.
- Administer medications and treatments in accordance with physician orders, CNO standards and JHH policies.
- Recognize, assess and respond to changes in resident health status in a timely manner.
- Monitor residents’ physical,



emotional and psychosocial wellbeing and communicate findings to the interdisciplinary team.
- Use clinical judgment and critical thinking to support effective symptom management and end-of-life care.
- Provide emotional support, education and guidance to residents and families throughout the end-of-life journey.
- Confer with residents and families regarding their care plans and satisfaction with care.
- Support families in understanding care goals and participating in care where appropriate.
- Work collaboratively with the interdisciplinary team including physicians, nurses, personal support workers (PSWs), volunteers and other professionals.
- Support and provide direction to Registered Practical Nurses, PSWs and volunteers in the provision of resident care as appropriate.
- Participate in team communication, care planning discussions and interdisciplinary meetings.
- Advocate for residents and families by promoting their rights to autonomy, dignity, respect, privacy and informed decision-making.
- Provide culturally sensitive and person-centered care consistent with hospice values.
- Document assessments, care provided and clinical observations accurately and in a timely manner in accordance with CNO standards and JHH policies.
- Maintain a protected, clean and supportive environment for residents, families, staff and volunteers.
- Follow infection prevention, health and safety and organizational policies and procedures.
- Report medication errors,



incidents and concerns to the Executive Director according to JHH policies and complete required documentation.

QUALIFICATIONS

- RN in good standing with the CNO.
- Bachelor of Science in Nursing (BScN) or education and experience that allows the RN to practice to full scope.
- Minimum five years’ experience in hospice palliative care or related clinical setting preferred.
- Completion of Fundamentals of Hospice Palliative Care and Comprehensive Advanced Palliative Care Education (CAPCE) considered an asset.
- Current Basic Cardiac Life Support certification.
- Demonstrated ability to provide compassionate, person-centered care consistent with hospice palliative care principles, including physical, emotional, psychosocial and spiritual support.
- Demonstrated critical thinking, clinical assessment, problem-solving, communication and conflict management skills.
- Demonstrated knowledge and experience in pain and symptom management and end-of-life care.
- Demonstrated ability to work effectively within an interdisciplinary team in a respectful manner.
- Effective time management, organization and flexibility.
- Ability to manage emotionally sensitive and complex situations.
- Ability to meet the physical demands of the essential duties of the position.
- Commitment to ongoing professional development.
- Ability to maintain privacy and confidentiality at all times.
- Demonstrated reliability and ability to maintain regular and consistent attendance.
- Experience in electronic medical records (EMR) and clinical documentation systems, such as InfoAnywhere.
- Proficiency in Microsoft 365 or similar electronic documentation systems.

Pay: $43.61-$48.46 per hour
