Woolwich Community Health Centre (WCHC), is a multi‑site health centre and requires a Nurse Practitioner (22.50 hours per week, 0.6 Permanent FTE, Temporary Contract for 6 months at our Wellesley site).
All WCHC Nurse Practitioners work to their full scope of practice within an inter‑disciplinary team to provide comprehensive primary health care to a registered client caseload. In addition to providing comprehensive primary care, the Nurse Practitioner will also engage in community‑facing programming aimed at reducing primary care unattachment rates across rural Waterloo Region and surrounding areas.

Centre is responsible for program and service delivery to clients who are residents of Woolwich, Wellesley, and Wilmot Townships.
Requirements
1. BScN with Primary Health Care Nurse Practitioner certificate and registered with the College of Nurses of Ontario in Extended Class.
2. Broad‑based nursing experience with 2‑5 years in primary health care setting.
3. Commitment to working as part of an inter‑disciplinary team that includes family physicians, nurse practitioners, clinical nurses, chiropodists, registered dietitians, counsellors, physiotherapists and community health team staff.
4. Skills and experience to fulfil major functions of role: wellness care; assessment, diagnosis and management of episodic illness; managing chronic disease; and group work.
5. Strong communication, organizational, and issue‑management skills.
6.
Excellent interpersonal and problem‑solving skills within the inter‑disciplinary team setting and with clients and community members.
7. Flexibility to work one evening per week as well as provide after‑hours on‑call on a shared schedule with 15 other health centre physicians and nurse practitioners.
8. Knowledge of or experience with health issues across the lifecycle. Specific expertise in geriatrics, palliative care, mental health, chronic disease management, maternal and infant/child health and familiarity with working in a rural setting would be considered an asset.
9. Strong familiarity with computers; prior experience with electronic medical records would be an asset.
10. Car required for home visits.

- Salary of $121,556 to $131,893 per year ($62 to $68 per hour)
