About the Job

We're looking for a patient care & provider relations coordinator to join our growing team at Rehab2Go.

Rehab2Go is a multidisciplinary rehabilitation company bringing physiotherapy, occupational therapy, chiropractic care, massage therapy, osteopathy, acupuncture, and mental healthcare to patients across Ontario, in their home or virtually. Rather than employing clinicians out of a central location, we build and support a growing network of licensed, independent clinicians who deliver in-home and virtual care within their own local community, setting their own hours and coverage area.

Our vision is to make integrated rehabilitation accessible for patients everywhere, with clinicians working together as a true care team. Whether we're fitting a custom brace, running a virtual physio session, or supporting a patient through psychotherapy, our mission stays the same: to deliver accessible, multidisciplinary rehabilitation care for every Ontarian we serve.

The Role

Reporting to the Head of Growth and Partnerships, the Patient Care & Provider Relations Coordinator plays a central role in supporting both the patient experience and the Rehab2Go clinician network.

This position serves as a key point of coordination between patients, healthcare consultants, referral partners, and the leadership team. The coordinator helps ensure patients receive timely access to care while supporting the recruitment, onboarding, engagement, and administration of Rehab2Go's growing network of rehabilitation professionals.

The ideal candidate is highly organized, people-focused, and comfortable balancing patient scheduling, provider communications, recruitment activities, and operational support within a rapid-paced healthcare environment.

Key Responsibilities

Patient Coordination & Scheduling

- Support patient intake and appointment scheduling for new and existing patients.
- Coordinate assessments, treatments, and follow-up appointments with clinicians.
- Serve as a primary point of contact for patients regarding appointments, scheduling changes, and general inquiries.
- Ensure a high-quality patient experience through timely and professional communication.
- Support referral management and facilitate efficient patient onboarding.
- Assist with maintaining accurate patient information within electronic medical record systems.

Provider Relations & Recruitment




- Support the recruitment and onboarding of rehabilitation professionals across Ontario, including physiotherapists, occupational therapists, chiropractors, massage therapists, psychotherapists, and other regulated health professionals.
- Assist with sourcing, screening, and coordinating interviews with prospective consultants.
- Coordinate consultant contracts, credential collection, professional licensing verification, and onboarding documentation.
- Maintain consultant records and provider databases.
- Serve as an ongoing resource and point of contact for Rehab2Go consultants.
- Coordinate regular communications and engagement activities with clinicians to strengthen relationships and retention.
- Represent Rehab2Go professionally and help build its reputation as a strong, people-first place to work.

Communications & Engagement
- Draft and distribute newsletters, updates, and communications to patients, consultants, referral partners, and staff.
- Support social, educational, and networking initiatives for Rehab2Go's provider network.
- Assist in developing communication materials that enhance patient engagement and promote Rehab2Go services.
- Coordinate marketing and outreach communications as required.

Administrative & Operational Support
- Maintain accurate records, contracts, and supporting documentation.
- Support reporting and operational tracking related to patient volumes, provider engagement, recruitment activities, and scheduling metrics.
- Manage the day-to-day operations of our Toronto office, including food and snack orders, office supplies, and equipment.
- Provide general administrative support, including printing, preparing materials, and other day-to-day office tasks as needed
- Plan and organize informational and social events for contractors, staff, and patients
- Help identify opportunities to improve processes, workflows, and service delivery.
- Support the implementation of technology and AI-enabled tools that improve efficiency and patient service.

Qualifications

Required





- 2-3 years of experience in healthcare administration, patient coordination, clinic operations, medical office administration, customer service, recruitment, or related fields.
- Exceptional organizational and multitasking skills.
- Robust written and verbal communication abilities.
- Demonstrated ability to work with patients, healthcare professionals, and external partners.
- Experience managing schedules, appointments, and competing priorities.
- High attention to detail and strong follow-through.
- Proficiency with Microsoft Office, Google Workspace, Notion, Airtable, Canva, and comfort using new digital tools

Preferred

- Experience in rehabilitation, allied health, healthcare, or community health environments.
- Experience supporting recruitment and onboarding activities.
- Knowledge of electronic medical records (EMR) systems.
- Familiarity with contractor management and credentialing processes.
- Experience working in a startup or rapidly growing organization.

What Success Looks Like

Within the first year, this individual will:

- Help maintain a smooth and responsive patient scheduling and intake process.
- Contribute to high levels of patient satisfaction and engagement.
- Support the recruitment and onboarding of qualified rehabilitation consultants across Ontario.
- Ensure provider contracts, records, and communications are managed efficiently and accurately.
- Strengthen relationships with consultants, helping Rehab2Go build a highly engaged clinician network.
- Improve operational processes that support growth, efficiency, and exceptional patient care.

Location

This is a full-time, in-person role based out of Rehab2Go's head office in Toronto. This isn't a remote or hybrid role.

Compensation The pay range for this role is $20-25/hour. This is a full-time position.
