Patient Scheduling & Care Coordination Specialist

Work Structure: Fully Remote

Location: Anywhere in Canada, Monday-Friday, with an 11:00 AM Eastern Time (ET) start. This corresponds to approximately 8:00-9:00 AM local time depending on your location and the time of year.

Employment Type: Full-time (40 hours/week)

Salary: $45,000-$52,000

About the Role

We’re looking for a highly organized and patient-focused Patient Scheduling & Care Coordination Specialist to join our team.

Think of this role as the virtual front desk of our healthcare program. You’ll be one of the key points of contact for patients when they need help navigating their appointments, particularly when an appointment needs to be rescheduled. You’ll manage daily appointment workflows, coordinate telehealth logistics, communicate directly with patients, and ensure our providers’ schedules remain organized and efficient.

This role is ideal for someone who is detail-oriented, empathetic, comfortable with technology, and energized by helping people. You’ll need to be able to balance warm, reassuring patient communication with a high degree of operational accuracy.

What You'll Do

- Manage patient appointment scheduling and rescheduling workflows across multiple platforms




- Communicate with patients by phone, email, and text to coordinate appointments
- Help patients navigate changes to their appointments and provide clear instructions for their next steps
- Coordinate telehealth logistics and ensure patients have the information they need to successfully attend their virtual appointments
- Monitor daily appointment workflows and proactively identify scheduling conflicts, gaps, or issues
- Maintain accurate and up-to-date patient and appointment information across internal systems
- Coordinate with clinical and customer success teams to ensure patients are appropriately scheduled
- Help keep provider schedules full, organized, and operating efficiently
- Follow established scheduling procedures and escalate issues when appropriate
- Provide a warm, professional, and patient-centered experience at every interaction
- Support evolving patient experience and administrative needs as the team and company grow, with additional responsibilities as required

- Strong interpersonal and communication skills
- A patient, empathetic, and professional approach to customer interactions
- Comfortable communicating with patients who may have questions, concerns,



or difficulty navigating technology
- Strong ability to manage multiple tasks, priorities, and platforms simultaneously
- Comfortable learning and working across multiple software systems
- Reliable follow-through and the ability to take ownership of tasks from start to finish
- Ability to work independently in a fast-paced, evolving startup setting
- Strong written and verbal communication skills
- A proactive mindset- you notice what needs to be done and take action

Nice-to-Haves
- Previous experience in healthcare, health-tech, telehealth, medical administration, or customer success
- Experience with patient scheduling or appointment coordination
- Experience working with electronic medical records (EMRs), scheduling platforms, CRM systems, or telehealth software
- Familiarity with virtual healthcare workflows
- Experience working in a startup or rapidly changing environment
