We are seeking a skilled, compassionate, and motivated Social Worker to join our collaborative team. This role will provide meaningful support to patients and families, help address social and community-based factors that impact health and well-being, and contribute to population health initiatives.

The successful candidate will be organized, collaborative, and driven to provide high-quality, person-centred support while working as an integral member of an interdisciplinary primary care team.

We are currently recruiting a Social Worker who will provide support, advocacy, and the coordination of services for clients with complex needs. This position will collaborate with Stony Plain Family and Community Support Services and other local human service agencies to ensure quality of service to clients.

Ability to manage high acuity and multiple competing service demands. Excellent knowledge of AISH, EI, Alberta works, Income support, OAS, ASB, social services and other financial programs.

A degree in social work, active or eligible registration with the Alberta College of Social Workers, plus extensive experience working with families and community agencies. This role is dynamic and involves co-location at both WVPCN offices and family physician offices, as well as ability to work remotely and in compliance with WVPCN Remote Working Policy (including separate, secure, safe workspace and high-speed internet connectivity). Must have own vehicle and valid driver’s license.

Education

- Bachelor's Degree (preferred)
- Registration with the Alberta college of Social Workers is required.

Job Type: Permanent Full-time Salary: $28.00+ per/hr - depending on education and experience.

Schedule: Monday to Friday
