As a Homecare Nurse (HCN) at WNHAC, you will be part of an interprofessional team providing primary health care services. Your role emphasizes health promotion, disease prevention, and comprehensive client assessments. Collaborating with other healthcare professionals, you will develop and monitor care plans, ensuring high-quality services tailored to individual needs.

Key Responsibilities:

- Carry out assessments and treatment in clients' homes
- Collaborate for comprehensive care and referrals
- Develop and evaluate client care plans collaboratively
- Support intake processes and medical history data entry
- Maintain accurate medical records and documentation

Requirements:

- Current RN registration with the College of Nurses of Ontario
- Experience in home care nursing preferred
- Knowledge of Indigenous culture and health values
- Willingness to learn about Indigenous practices
- Valid driver’s license and willingness to travel
