Are you a passionate and experienced social worker who is committed to supporting mental health, wellness, and healing within Indigenous communities? Squamish Nation is seeking a Wellness Coordinator/Social Worker to join the Yúustway Health & Wellness team. Reporting to the Manager, Kal'númet Primary Care, the Wellness Coordinator/Social Worker is a registered clinician and valued member of our interdisciplinary health and wellness team. This role provides culturally safe, holistic, and client-centered support to Squamish Nation members and families through outreach, counselling, advocacy, care coordination, and case management services. Working closely with healthcare providers, community partners, and families, the Wellness Coordinator helps clients navigate complex systems while supporting improved health and wellness outcomes.

What You’ll Do

- Conduct clinical assessments, treatment planning, case formulation, goal setting, and progress evaluations with clients.
- Provide education, short-term individual counselling, and group support using culturally safe and harm reduction approaches.
- Deliver outreach services and build meaningful connections with community members who may be difficult to engage.
- Complete mental health and substance use intakes and develop individualized Wellness Plans.
- Provide short-term counselling, consultation, suicide risk assessments, family assessments, and mental health assessments.
- Participate in interdisciplinary case management meetings and collaborate on care planning for clients experiencing mental health and substance use challenges.
- Support clients in navigating healthcare systems, including treatment programs, hospital services, specialist appointments, and community resources.




- Coordinate and support access to temporary or permanent housing resources for clients when appropriate.
- Collaborate with families, healthcare professionals, Squamish Nation departments, and community partners to provide wraparound support.
- Support the development and delivery of health education related to mental health, substance use, and concurrent disorders.
- Maintain accurate client records and documentation within the Electronic Medical Record (EMR) system while ensuring confidentiality and compliance with professional standards.
- Support community events, program development initiatives, and wellness activities that reflect Squamish Nation culture, values, and traditions.

What We’re Looking For

- Master's or Bachelor's Degree in Social Work or a related discipline from a recognized post-secondary institution.
- Registration in good standing with the BC College of Social Workers.
- Minimum 3 years of recent clinical social work, therapeutic counselling, mental health, substance use, or concurrent disorder experience.
- 3 to 5 years of related experience in counselling, case management, outreach, system navigation, advocacy, and community-based support services.
- Experience supporting individuals experiencing mental health, substance use, and concurrent disorder challenges.
- Experience working in or alongside a First Nations community and a strong understanding of Indigenous health and wellness approaches is preferred.
- Knowledge of Indigenous Cultural Safety and humility, or a willingness to further develop these competencies.
- Strong communication, relationship-building, and collaboration skills with diverse clients, families, and service providers.
- High level of emotional intelligence, sound judgment, patience,



professionalism, and integrity.
- Strong organizational and time management skills with the ability to manage a complex caseload and competing priorities.
- Proficiency with Microsoft Office applications and experience using Electronic Medical Record (EMR) systems or comparable databases.
- Knowledge and understanding of Skwxwú7mesh Úxwumixw history, culture, customs, land, and People is preferred.
