Join our team as a full time Nurse Practitioner, where you will support primary health care with a focus on wholistic and culturally relevant approaches. You will work collaboratively with an interprofessional team, providing comprehensive assessments and client-centered care.
In this role, you will function independently within the Extended Class scope of practice while engaging with health professionals and community members. Your responsibilities include diagnosis, treatment planning, and continuous evaluation of client care plans.
You will also lead clinical initiatives and contribute to quality improvement processes while embracing Indigenous health ideals.
Key Responsibilities:
- Conduct comprehensive assessments and prescribe treatments within NP scope
- Monitor and coordinate client care plans for optimal health goals
- Collaborate with team members for wholistic care delivery
- Provide mentorship and support clinical leadership initiatives
- Contribute to quality improvement and evaluative practices
Requirements:
- Current Extended Class registration with College of Nurses of Ontario
- Proven ability to work independently and within teams
- Effective communication with diverse populations
- Understanding of wholistic health in Indigenous contexts
- Valid driver's license and willingness to travel
