Key Responsibilities:
• Develop evidence-informed care plans for rehabilitation goals
• Collaborate with multidisciplinary teams for optimal patient outcomes
• Mentor and lead within your team as needed




• Utilize e-charting and digital communication effectively
• Educate patients on self-management strategies

Requirements:
• Active provincial regulatory college license in positive standing
• 1–2+ years of outpatient LTC experience preferred
• Exceptional interpersonal skills with a patient-first approach
• Valid Vulnerable Sector screening check
• Proof of Tuberculin clearance required
