Shared Health is seeking a Service Coordinator - ACT to join the HUB, FACTT and ACT teams supporting individuals with severe mental illness. You will provide case management and coordinate a multidisciplinary team to ensure wrap-around community services.
You will work under supervision to assess eligibility and plan client care. Requirements include a minimum of four years in community health, a relevant health/human service degree, and active regulatory registration.
