Key Responsibilities:
• Complete in-home functional assessments and mobility evaluations
• Develop individualized, evidence-based treatment plans
• Recommend assistive devices and environmental modifications




• Provide follow-up visits for progressive functional goals
• Educate clients and families to promote independence

Requirements:
• Registered with the College of Occupational Therapists of Ontario
• Experience in home and community care preferred
• Robust skills in functional assessments and documentation
• Ability to work independently within service area
• Access to a reliable vehicle is an asset
