Key Responsibilities:
• Provide drug therapy optimization in a patient-centered setting • Collaborate with healthcare professionals for drug-related solutions • Oversee medication reconciliation at patient admission • Verify and manage pharmaceutical orders effectively • Participate in clinical and educational program development
Requirements: • Licensed Part A pharmacist with Ontario College • Solid communication and relationship-building skills • Demonstrated ability in managing competing priorities • Proficiency in Cerner and Microsoft Office Suite • Previous hospital pharmacy experience is beneficial
