Become a vital part of St. Leonard’s Community Services as a Community Case Worker in the Reintegration Program. This 18-month contract focuses on supporting individuals with criminal justice system involvement.
The role involves assessment, case management, and developing individualized support plans for community-referred participants. You will play a key role in network building, connecting clients with proper services, and ensuring effective day-to-day support and discharge planning. Besides, experience with mental health and addiction challenges is essential for success in this position.
Key Responsibilities: • Conduct assessments and manage individualized support plans • Network with community partners to enhance service delivery • Liaise with court, police,
and ministry officials • Work independently, multitasking within community contexts • Represent SLCS while focusing on participant reintegration
Requirements: • Post-secondary education from an accredited institution • 1 - 3 years in social services settings • Level C First Aid/CPR training • Valid G Class Driver’s License • Bilingual proficiency in English and French preferred
