Key Responsibilities: • Coordinate and supervise Community Support Workers in the field • Develop and manage effective Care Plans for patients • Monitor employee performance and health policies • Promote Community Health Services and foster public relations • Guide workplace health and safety initiatives
Requirements: • RN or RPN degree or diploma • 3-5 years related job experience • Registration with the College of Nurses of Ontario • Proven leadership and case management skills • Current CPR Certification
