Responsibilities Observe, document, and promptly report changes in residents'' physical, emotional or cognitive status to the primary nurse to support ongoing assessment and care planning Complete all documentation on time, andin accordance withcorporate policies, ensuring that it is both current andaccurate.

Assistresidentwith activities of daily living (ADLs) including bathing, dressing, grooming, mobility, toileting, and feeding, while promoting dignity and independence.

Assistin the safe transfer, positioning, and mobility of residents using proper techniques and equipment, andin accordance withfacility policies Provide carein accordance withthe Fixing Long

Term Care Act, 2021 (S.O.2021, c.39, Sch.1), best practices, corporate policies, and procedures.

Participate in agency risk management program related to resident care.

Participate in quality improvementprogramrelated to resident care.

Participate innew staff/student mentoring as assigned and promote educational opportunities to enhance their education.

Participate in departmental, Interdisciplinary

Teamand resident safety meetings.

Promote good understanding and teamwork between shifts,



registered and unregulated staff Adhere to all health and safety procedures, including: Use all equipmentin accordance withmanufacturer''s instructions.

Follow established safety procedures for lock outs.

Wear proper safety equipment and PPE when necessary.

Requirements Must be at least 18 years of age.

Proficient verbal and written English communication skills.

Abilityto speak a second language is considered an asset.

Must meet one of the following criteria: Successful completion of a Ministry of Education approved PSW Training Program (minimum 640 hours) from a Community College or a school approved by the National Association of Career Colleges OR RN/RPN students with successful completion of Year 1 Experience working as a Facility PSW is considered an asset.
