Position Overview

We are seeking a passionate and community-minded Community Wellbeing Promoter who is dedicated to helping individuals and communities thrive physically, mentally, emotionally, and socially.

This role focuses on health promotion, community development, social prescribing, and wellness programming. You will work directly with individuals, groups, community partners, and healthcare providers to create meaningful opportunities that support healthy and connected lives.

Key Responsibilities

- Lead engaging, inclusive group wellness programs, including workshops,physical activity programs, educational sessions, and community events.
- Work directly with individuals and priority populations to improve wellbeing and promote healthy lifestyles.
- Develop and adapt health promotion and community development initiatives based on local needs and strengths.




- Coordinate and build up Gateway's Social Prescribing Program by connecting individuals to community services, programs, and activities that support mental health, wellness, and social inclusion.
- Build and maintain collaborative relationships with community organizations, healthcare providers, schools, municipalities, and other local partners.
- Deliver outreach activities, presentations, and public education initiatives that increase awareness of health and wellness resources.
- Foster welcoming, safe, and inclusive environments where participants feel supported and empowered.
- Share evidence-informed health information in a clear, accessible, and non-judgmental manner.
- Document services, activities, and client interactions accurately and in a timely manner using the Electronic Medical Record (EMR) system.
- Support evaluation, reporting, and continuous improvement efforts related to community programs and wellness initiatives.

Qualifications

Required

- Diploma or degree in Social Services, Social Work, Community Development, Public Health, Mental Health, Health Promotion, or a related field.
- Experience delivering or supporting group-based programming in community settings.
- Strong communication, facilitation, and relationship-building skills.
- Ability to work independently and collaboratively within a multidisciplinary team.
- Strong organizational and time-management skills.
- Comfort using technology and learning digital systems for documentation, program tracking, and resource sharing.
- Valid Ontario driver's license and access to a reliable vehicle.

Assets
- Experience working in community wellness, social services, health promotion, or mental health settings.




- Experience with food security or food insecurity programming.
- Experience working within a Community Health Centre.
- Knowledge of the Social Determinants of Health.
- Experience working with vulnerable, marginalized, or underserved populations.
- Experience using Electronic Medical Records (EMR) systems.
- Experience coordinating volunteers, partnerships, or community events.
