· Perform medication reviews for appropriate indication, dose, frequency, drug administration, drug interactions, allergies, formulary compliance,



cost effectiveness and duplication of therapy.

· Counsel patients and/or health care providers on medication reviews and provides additional related pharmaceutical information as required.

· Act as a patient advocate to ensure that patients receive optimal health care per best practice guidelines.

· Patient interviewing and history taking; consultation with other team members as needed to develop/review plan of care.

· Effectively maintain healthy working relationships within the collaborative, multi-disciplinary environment.

· Ensure appropriate treatment is delivered by maintaining proper patient records and assessment notes

· Contribute to developing new programs by identifying, recommending and developing resources and materials and by participation in such programs: (such as medical directives, fall-prevention programs, and memory clinic program)

· Abide by and work within policies and procedures of the HHC

· Maintain confidentiality in accordance with the Personal Health Information Act (PHIPA)

· Perform miscellaneous duties as required

· Provide diabetes education to patients by using best practices from the Canadian Diabetes Guidelines (CDE preferred)

· Aid patients in smoking cessation by participating in the STOP program, using best practices from the TEACH program.

· Must be licensed and in good standing with the Ontario College of Pharmacists, expected to adhere to the Standards of Practice for Pharmacists with the Ontario College of Pharmacists

· Baccalaureate of Science in Pharmacy, Pharm D preferred.





· Minimum 2-5 years related or applicable experience in a community, hospital or primary care setting a must

· Demonstrated experience in a Primary Care setting would be an asset

· Additional training or credentialing in pharmacotherapy (e.g. Certified Geriatric Pharmacist, Certified Diabetes Educator, Certified Asthma educator, etc.) is considered an asset

· Exceptional understanding of current, evidence-based and best practice pharmacy guidelines

· Participate in individual and group education sessions for patients and providers

· Experience working in an electronic medical records environment; Experience with Accuro EMR is an asset

· Multi-tasking skills related to the delivery of effective primary care including the ability to share information and teach while treating a patient

· Demonstrated ability to work effectively in a multi-disciplinary workplace.

· Demonstrated commitment to and knowledge of community- based health care.

· Awareness of and sensitivity to the health issues of a diverse community

· Excellent communication, interpersonal, organizational and administrative skills including computer literacy

$41.50 - $47.00/hour (based on experience)

15 hours/week (2 days @ 7.5 hours)

Pay: $41.50-$47.00 per hour
