- Lead resident move-in, transfer, and move-out processes
- Provide counseling and assess psychosocial needs
- Develop and implement individual social work services
- Support advanced care planning conversations
- Manage correspondence and feedback tracking in homes

Requirements:

- Registered Social Worker per provincial regulations
- Minimum three years’ healthcare or social services experience
- Knowledge of Long Term Care Act preferred
- Robust communication and organizational skills
- Experience in customer service or hospitality is an asset
