The Social

Worker II is responsible for the assessment and treatment of patients, as well as the development, implementation, and evaluation of treatment programs in accordance with professional practice standards. Our team of passionate health care professionals is committed to enhancing the quality of life of those affected by illness or injury by providing comprehensive support and care from diagnosis, through treatment, and back to community reintegration. You will be joining a progressive team of Allied Health professionals, including Social Workers, Social Work Assistants, Housing Support Workers, Transition Coordinators, Occupational Therapists, Physiotherapists, and more, who collaborate continuously to provide exceptional care and mentorship opportunities.

Description

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications

Recent work experience as a Social Worker (within the past three years) in a hospital setting, preferably on a medicine or surgery unit. Equivalent experience will be considered, including experience working in non-hospital medical settings or on crisis-response teams. Solid skills in crisis intervention, psychosocial assessment, and complex planning. Demonstrated ability to make timely decisions and prioritize effectively in a fast-paced environment. Excellent verbal and written communication skills, including proficiency in clinical documentation. Ability to work both independently and collaboratively within an interdisciplinary team. Knowledge of Edmonton-area community resources and referral pathways.

Experience working with individuals facing houselessness, poverty, substance use, and mental health challenges. Demonstrated culturally safe and trauma-informed practice.

Preferred Qualifications

Strong working knowledge of hospital electronic medical records systems (e.g., Connect Care).
