The Hospital Liaison works with the Saskatchewan Health Authority, NIHB, community partners, and BRT6HC programs to ensure wholistic, coordinated care throughout the healthcare journey. Responsibilities include advocacy, health system navigation, discharge planning, and client support, while fostering communication
