Bayshore Healthcare is seeking a Nurse Case Manager to support patients living with HIV, helping with enrolment, coverage navigation, and timely drug reimbursement. You will assist physicians with program paperwork, follow up on submissions for private and provincial formularies, and coordinate monthly injections to ensure continuity of care.You will liaise with third-party providers and provincial bodies, schedule diagnostic tests, perform Welcome Calls, and maintain accurate records.#J-18808-Ljbffr
