Job Summary
TheCase Manager at Phoenix Society facilitates the recovery and capacity-buildingof clients to maximize their social and economic potential and support theirgoals for successful independent living in the community.
Theposition is responsible for: case management, including the facility residentorientation, assessment, supportivecounselling, and group facilitation; client support;and community building. The Case Manager connects clients to needed services withaddiction counsellors, employment counsellors and others at the Society, aswell as with community organizations.
Responsibilities
CaseManagement
- Providecase management services to assigned caseload of clients
- Observesclients, identifies and assesses potential emergency situations, and developsshort-term strategies to deal with such situations. Reports medical,behavioural, and other problems as required. Provides feedback and inputregarding clients’ needs, performance, and progress
- Maintainand update client case management files on the database system, ensuring thatmonthly reporting deadlines are met
- Providessupportive counselling, demonstrations, modeling, and behaviour management toclients, their families, and their personal networks, to assist with thedevelopment of physical, social, emotional, and life skills such as maintainingpersonal hygiene, housekeeping, meal planning and preparation, meetingfinancial obligations, making and keeping appointments, and interpersonalskills
- Completesand maintains related records and documentation such as care plans, progressreports, statistics, and daily logs
- Identifyand prioritize clients’ needs in the following areas: employment; financialsupport; substance use; criminal justice involvement; family history; etc.
- Assistin coordinating routine medical care and collaborating with on-site medicalteam
- Assistin liaising with psychiatrists, mental health teams, etc. as part of case worksupport services
- Attendand participate in Interdisciplinary team meetings and collaborate with otheron-site professionals

ClientServices

- Teachclients effective social and interpersonal skills through daily groups andthrough the tasks of appropriate daily living and social activities
- Contributeto maintaining a therapeutic environment following therapeutic communityprinciples which encourage awareness of interdependence of community membersand the value of responsible concern for oneself and others
- Prepareintake and admission documentation for review with new clients in the HousingProgram prior to the client’s move-in date
- Welcomeand orient incoming clients to Transitional and Supportive Housing Programs.Review the Resident Handbook with clients and assist them with move-in needssuch as bedbug protocols, unpacking, getting familiar with the building, healthand safety protocols, community resources and staff locations
- Formand maintain positive and effective collaborative relationships ofaccountability with clients in the areas of education, employment, recovery,volunteer participation and independent living
- Activelyinvolve the client in action planning and encourage them to take responsibilityfor the outcomes they are striving to achieve
- Provide crisis intervention when necessary
- Assistin monitoring client’s activities within residential buildings to ensure thatareas are appropriately used for their purpose by visiting floors and commonareas and viewing video camera feeds as required
- Connectclients to relevant community resources, services, and opportunities
- Assistin bridging clients to long-term un-serviced housing
- Actas an advocate for the client during their stay and maintain contact afterdischarge to provide follow-up support if possible
- Conductingmonthly suite inspections, move in and move out Inspection as required
- Act as representative of the programin the OHS committee
Groupfacilitation
- Facilitatesupport groups and workshops focusing on life skills, health and wellness,recovery planning, harm reduction, communication, personal financialmanagement, stress management, etc.
- Determinethe needs and composition of counselling groups and design or adapt a processtailored to clients’ needs
- Workto improve groups and client meetings by adjusting interventions, strategies,and approaches to meet group members’ needs
- Helpgroup members establish how they will treat each other. Facilitatecommunication among group members
- Encouragethe engagement and participation of all group members
- Evaluategroup outcomes and incorporate feedback into future learning situations
CommunityRelations
- Assistin liaising with treatment centres, support recovery programs, addictions andmental health workers, shelters, primary care clinics, correctionalinstitutions, police, hospitals, and outpatient clinics to reach prospectiveclients to improve continuity of care
- Maintaincurrent knowledge of social, economic, recreational, and educational resourcesin the community
Qualifications
- Post-secondarydiploma (e.g., 2 to 3 years) in case management, social work, psychology, etc.
- 1to 3 years of experience providing case management support in the community social servicessector and working with marginalized populations, substance use services,mental health, corrections, etc., or equivalent
- Satisfactorycompletion of a criminal records check with vulnerable sector screening isrequired
- Satisfactorycompletion of Tuberculosis Screening form is required
- Satisfactory completion of Nonviolent crisis intervention (NVCI)
- Satisfactorycompletion of Employee Immunization Record form is require
Skills & Knowledge
- Excellent knowledge of the issues pertaining torecovery, substance use, concurrent disorders, homelessness, and addictionscounselling
- Good knowledge of facilitation,mediation, and conflict resolution techniques
- Highlevel of integrity and dependability with a strong sense of urgency andresults-orientation
- Strongcase management, teaching/training, clinical support, and consultation skills
- Strongskills in Windows, Microsoft Word, Excel, and database management systems
- Strongproblem solving, analytical, and creative skills with the ability to exercisesound judgement and make decisions based on accurate and timely analyses.
- Stronginterpersonal skills and ability to communicate effectively in writing andverbally in English
