1 year in primary care, including:o Assessment and triageo Chronic disease management (e.G., diabetes, hypertension, asthma)o Medication/treatment administration (per medical directives)o Symptom monitoring and clinician updateso Women’s health screening and contraception counselingo Travel health advice and pre-travel immunizationso Pediatric and/or perinatal experience (asset)- Strong assessment, health promotion/education, counseling, and community developmentskills- Ability to work effectively on an interdisciplinary team- Ability to work flexible schedule, including evenings and weekends.- Additional languages, preferably Spanish or Arabic, and EMR/Practice Solutions experienceare assets.Working Conditions & Physical Requirements:- Incumbent will be expected to work some evening and/or weekend hours based on projectdemands.Note:- This is a bargaining unit position with UFCW.
