Lead primary care initiatives with Pinecrest-Queensway Community Health Centre as a Nurse Practitioner dedicated to falls prevention in Ottawa. This part time position involves direct patient support and comprehensive health management. Working 21 hours weekly, you will provide individualized assessments and treatment for clients, addressing both acute and chronic health needs.
Collaborating with physiotherapists and social services, you will enhance community engagement and education on health promotion.
Your role supports marginalized populations facing barriers to care, ensuring inclusivity in service delivery. Key Responsibilities:
Act as the Most Responsible Provider for a panel of clients
Diagnose and manage health conditions across community settings
Collaborate with diverse health professionals for client care
Conduct thorough health assessments and create care strategies
Promote health literacy and resources in the community Requirements:
Master’s degree in Nursing, active CNO Extended Class registration
Two years of relevant Nurse Practitioner experience preferred
Commitment to community-based health initiatives
Proficient in electronic medical records and office software
