We're seeking a Home Support Supervisor to oversee community-based nursing care in Peel, North York, Scarborough, York, and Toronto. As a Registered Practical Nurse, you'll lead and develop your PSW team. Your primary focus will be on patient assessments and setting care goals while ensuring your team is well-trained and supported. You will manage staff performance and maintain awareness of key indicators that affect service quality and patient satisfaction. Key Responsibilities:
Conduct home visits to assess patient needs
Formulate and implement care plans with the team
Manage staff through performance evaluations
Record and analyze feedback for service improvement
Address patient concerns efficiently Requirements:
Must be registered with CNO in positive standing
At least three years of direct supervisory experience
Valid driver's license and access to transportation
Solid solution-oriented interpersonal skills
Proficient in English communication, additional languages are a plus Lead by example and drive healthcare excellence as a Home Support Supervisor.
