Deliver high-quality, patient-centered medical care
Achieve a minimum roster of 650 patients
Collaborate within a supportive healthcare team
Participate in after-hours patient coverage
Maximize use of the MyCare app for care coordination Requirements:
Must hold an Independent Practice Certificate from CPSO
Obtain medical liability insurance (CMPA or equivalent)
