Join MyPotentia as a C.A.R.E Connector in Calgary, AB, providing essential support for individuals with FASD. This hybrid role focuses on client-centered advocacy and comprehensive care for adults.

As a full-time C.A.R.E Connector, you will engage in assessment, case management, and client advocacy. You will collaborate with the Intake Team to create individualized C.A.R.E plans, ensuring they evolve with client needs. Your role will emphasize navigating systems, coordinating services, and upholding documentation standards for client care.

Key Responsibilities:
• Develop and review individualized C.A.R.E plans with clients
• Provide ongoing support as per client-directed plans
• Advocate for clients’ needs in their communities




• Coordinate services with multiple agencies
• Maintain documentation of care plans and progress

Requirements:
• Diploma in Health, Social Services, or related field
• Understanding of FASD and its community impact
• Solid communication and interpersonal abilities
• Compassionate and empathetic approach
• Must have a driver’s license and vehicle insurance
