Position Overview
Closing the Gap Healthcare (CTG) is dedicated to providing Canadians with the highest level of care across a wide range of home and community settings. We are seeking a passionate Social Worker who thrives on independence and flexibility and who will craft a schedule that fits both personal and client needs while working face‑to‑face with adult clients. Skills Needed for Success
Master’s degree in Social Work from an accredited university
Knowledge of multiple community resources in Toronto
Current active registration with the Ontario College of Certified Social Workers (SWW) in good standing
Valid driver’s license and reliable insured vehicle
Completed vulnerable sector screening (VSS) within 12 months of hire (or in process) and a current criminal records check
