This shared role entails direct engagement with residents and families, where you will assess psychosocial needs and provide essential support in care transitions. Key Responsibilities:
• Perform psychosocial assessments and counselling
• Assist residents during health changes and end-of-life transitions
• Collaborate in care planning and conferences
• Inform residents of community resources and financial aid
• Champion residents’ dignity and rights

Requirements:
• Degree in Social Work (BSW or MSW)
• Certification eligibility with the BC College of Social Workers
• Experience in long-term or seniors care preferred
• Robust skills in communication and relationship building
• Valid driver’s license for site travel
