Key Responsibilities:
• Assess client needs and develop care strategies
• Provide direct nursing services in the community

• Lead and participate in case management initiatives
• Document care processes accurately and timely

Requirements:
• Registration with the College of Registered Nurses of Alberta
• At least 2 years experience in community or home care
• Current BCLS-HCP within last year
• Proficient computer skills for case management
• Ability to stand and lift clients when required
