Position Overview:
We are currently seeking an Occupational Therapist to join our dynamic interdisciplinary team. This role supports patients and families across our community through assessment, intervention, education, and collaboration with healthcare partners. The Occupational Therapist will support patients in achieving optimal function and independence in their daily lives through assessment, intervention, and education. As an integral member of the interdisciplinary team, the Occupational Therapist collaborates with patients, families, and healthcare providers to develop personalized care plans that address physical, cognitive, and psychosocial needs across the lifespan.

Roles and Responsibilities:
Primary Care Integration
Attachment Efforts:

Support patients who do not have primary care to be referred to the attachment team with the OCEAN e-referral.

Access Efforts:

Be an ambassador to the groups and workshops offered by HFHT by being aware of current offerings, handing out promotional materials and promoting the groups and workshops to existing, new and future patients.

Contribute to improved access and continuity of care across local healthcare services.

Quality & Performance Improvement

Ensure services reflect current best practices and ministry guidelines

Lead or support program evaluation and quality improvement initiatives

Monitor and evaluate outcomes to inform service planning

Promote a safe, inclusive, and respectful care environment

Address patient concerns with sensitivity and professionalism

Foster a culture of patient engagement and shared decision-making

Resource Optimization

Support the effective use of resources to meet service demands

Identify gaps and help develop processes that improve efficiency and capacity

Leadership

Model the HFHT’s values of collaboration, inclusivity,



and excellence

Support and mentor team members to maximize team effectiveness

Champion equity, diversity, and psychological safety in the workplace

Serve as a leader, advocate and ambassador of occupational therapy in primary care

Occupational Therapy Related Duties and Responsibilities:

Conduct comprehensive assessments to identify physical, cognitive, and functional needs.

Develop and implement individualized treatment plans to support daily living and independence.

Provide education and strategies for areas such as managing chronic conditions, mental health, cognition, injury prevention, home safety, functional mobility and more.

Collaborate with interprofessional team members to support coordinated, patient-centered care.

Facilitate individual and group programs focused on wellness, self-management, and functional improvement.

Recommend assistive devices, home/workplace modifications, and community resources.

Document care and progress in the electronic medical record (EMR).

Support quality improvement initiatives and contribute to program development.

Assist patients with navigation of healthcare and community support systems.

Qualifications and Skills:
Required:

Master’s of Science in Occupational Therapy (MScOT) or equivalent

Registered and in good standing with the College of Occupational Therapists of Ontario

Up to date professional liability insurance

Demonstrated competence across the full scope of occupational therapy practice, including assessment, intervention, goal setting, care planning, and documentation.

Demonstrated clinical reasoning and therapeutic skills

Ability to work independently in a collaborative, interdisciplinary setting

Experience in community-based care and system navigation

Program development implementation and evaluation experience





Reliable transportation (vehicle and G license)

Minimum 2 years of relevant experience in a primary care or community setting.

Preferred:

Registration as an Ontario Assistive Devices Program (ADP) authorizer for mobility devices such as walkers and wheelchairs

Additional training in chronic disease management, pain management, seniors care, home safety, or health promotion activities

Group facilitation experience and program planning expertise

Familiarity with Electronic Medical Records (EMRs) such as Telus PS Suite and Oscar

Additional Knowledge and Skills

Strategic thinker with a proactive, solution-focused mindset

Strong interpersonal and conflict-resolution skills

Clear communicator with the ability to build consensus

A strong understanding of applying legislation in practice (i.e., PHIPA)

Committed to evidence-based practice and continuous improvement

Classification: Non-Union
Multi-Site: Yes (some travel across the region to different HFHT practices and hubs, equipment vendor locations and patient homes, may be required)
Employee Class: Full time
Schedule: Monday-Friday; 40hrs/week
