Key Responsibilities:
• Coordinate cross-site MDT meetings for effective patient management
• Maintain and monitor Proactive & Community Matron’s communications
• Ensure accurate patient data tracking and reporting




• Organize meeting documentation, including minutes and agendas
• Assist in implementing clinical protocols and service improvements

Requirements:
• Solid experience with office programs and RiO
• Excellent data entry and administrative skills
• Ability to work effectively in a multidisciplinary setting
• Knowledge of patient care processes and quality assurance
