As part of the Healthy Aging program, you will utilize your nursing skills to enhance patient care and promote well-being. This role requires current registration with the College of Nurses of Ontario and a preference for Gerontology certification. Your practice will be centered around patient assessment, discharge planning, and effective teamwork.

Key Responsibilities:
• Use the nursing process to provide quality patient care
• Collaborate with healthcare professionals in a multidisciplinary approach




• Prepare and maintain accurate nursing documentation
• Engage in leadership to foster family-centered care
• Participate in required training to ensure safety and efficacy

Requirements:
• Active registration with the College of Nurses of Ontario
• Current BCLS certification required
• Comprehensive knowledge in gerontology nursing
• Proven problem-solving and interpersonal skills
