The MDT Co-ordinator role aims to ensure the smooth running of the MDT case conferences by ensuring the MDT meetings run effectively and are well structured. All patients are treated and discussed according to evidence-based protocols. While providing an effective and confidential administrative service to the Proactive team & Community Matron Team.By liaising and networking with MDT members and Enfield GPs, the post holder will promptly ensure that patients are referred to the Enfield MDT, enabling the patient to be discussed and referred to the appropriate team. The post holder is responsible for capturing all patient information, ensuring that up-to-date, relevant information is recorded on the database, and providing feedback to the referring services.The post holder will coordinate cross-site MDT meetings, ensuring the effective running and structure, ensuring all necessary information is available to enable clinical decisions, and ensuring that decisions are documented according to local protocols. He/she will assist service managers in facilitating service improvement initiatives to improve the experience and outcome of patient care.
Support the team of Enfield Proactive & Community Matrons team.
Assist in monitoring and updating the patients on the Proactive & Community Matron caseload.
Monitor and maintain the Proactive & Community Matron’s mailbox via NHS.net and post for assigned staff/ matron and to action accordingly.
To ensure all administrative duties about the Proactive Care & Community Matron Team are undertaken, ensuring appropriate records are maintained.
To be able to provide cover for all services of the ILT as and when required.
Typing and data entry using general office programs and Trust specific Computer Programs, i.e. RiO.
To act as a secretary for meetings, organising dairies, taking and distributing minutes, including the coordination of all paperwork required for all meetings promptly, i.e. Agenda.
Implementing staff rota’s and update ERoster as appropriate.
Responsibility for monitoring and actioning the Proactive Care & Community Matron Team’s information data reports on a weekly/monthly/quarterly basis.

Ensuring that patient information is distributed and available at the MDT meetings in line with the agreed process.
Note and track the progress of actions agreed at the case conferences /MDT meetings.
Support the implementation of the case conference/ MDT meeting approach to all GP practices in Enfield.
Develop relationships with Practice Managers, GPs and other MDT members to support the rollout.
Chair the conferences where required and consider how to improve the process to optimise MDT members' involvement .

To take accurate formal minutes, track actions, and ensure documentation is updated accordingly.
Communicate effectively with members of the integrated Locality Team, patients and external organisations using appropriate methods and providing appropriate responses.
To attend each MDT teleconference either in person or virtually and capture the actions agreed.
To liaise with GP practices and the MDT members to ensure the relevant patient data is received and distributed promptly for each case conference by the flow chart .
To advise the GPs and MDT members of the date and time of each MDT meeting, noting attendance.
To advise the GPs and MDT members of the attendees at each MDT meeting
To keep an up-to-date contact list for all members of the MDT.
Help with the introduction and changes to pro-form to ensure all patients are discussed and treated appropriately, and outcomes are recorded and reviewed. Ensuring patients’ diagnoses, investigations, and management and treatment plans are completed and added to the patient’s notes.
Ensure there is a clear definition of roles and responsibilities for MDT members.
Where appropriate, ensure there is a room and telephone available in a confidential area if dialling in from various location is required.
To be responsible for the collation, collection and inputting of the patient activity data required to monitor and quality assure the Enfield Integrated Locality Team Services.
To track the actions on an Excel tracker and follow up with the owners of the actions to track progress.
To maintain a history of the patient information provided to the MDT.
To support the rollout of the risk stratification tool to practices, completing any evaluation and assessment required.
To provide information to finance to verify that individual patient care plans have been created and payments are due.
To assist in the production of reports as required, e.g. tracking case conference outcomes.
To organise any other MDT meetings as required, e.g. review/evaluation meetings.
To elevate any issues to the Chair/Project manager as appropriate
To ensure own actions contribute to the maintenance of a quality service provision.
To be cognisant of IG requirements and provide advice and support to MDT participants to be compliant.
To provide input into the development of the OPAU.
The post will be regularly reviewed, and additional duties may be assigned depending on how the post develops.
Ensure telephones are answered promptly to facilitate efficient communication during office hours.
To type and distribute clinical and general correspondence per Trust standards.
To have responsibility for input and maintenance of good quality data.
To organise and have responsibility for all communication hardware and software agreed with the lead members of the MDT.
