Enhance the lives of residents as the Resident & Family Experience Coordinator III with Sienna Senior Living. This role focuses on creating meaningful connections and providing vital support in long-term care.

As a vital part of our community, you'll lead the resident move-in process, guide families, and collaborate with an inter-professional team. Your expertise in social work will help address psychosocial needs and overall well-being, making a significant impact on residents’ lives every day.

Key Responsibilities:
• Lead resident move-in, transfer, and move-out processes
• Provide counseling and assess psychosocial needs
• Develop and implement individual social work services
• Support advanced care planning conversations




• Manage correspondence and feedback tracking in homes

Requirements:
• Registered Social Worker per provincial regulations
• Minimum three years’ healthcare or social services experience
• Knowledge of Long Term Care Act preferred
• Robust communication and organizational skills
• Experience in customer service or hospitality is an asset
