This program provides $25,000 in grant funding to eligible RPNs/RNs/ Nurse Practitioners in exchange for a two-year full time commitment. The Registered Nurse, with the Geriatric Resource Nurse Outreach Program, will be part of an exciting team that supports patients and caregivers by providing in-home comprehensive geriatric assessment for at risk older adults living in the community. The applicant will work closely with Geriatricians and local Geriatric providers in the community.,.
Essential Qualifications Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA
Current R.N. certificate of competence with the College of Nurses of Ontario
Valid CPR certificate
Previous relevant inpatient or community-based experience (examples: rehab, Outreach)
Two years recent experience in geriatric assessment (Including Geriatric Medicine and/or Geriatric Psychiatry)
Excellent problem-solving skills and ability to prioritize in situations of varying degrees of complexity
Ability to function as an independent practitioner and a highly effective team member




Ability to gather medical and social history and clinical data to inform care planning and clinical decision making
Fundamental understanding of physiological and biopsychosocial mechanisms of the aging processes, age-related changes to function, and the impact of frailty
Ability to conduct accurate analysis of assessment findings and clinical information and to integrate assessment findings across geriatric assessment domains, such as falls/mobility, polypharmacy, continence, pain, cognition, and mood
Demonstrated expertise in treatment, education, goal setting, and future planning with older adults and their identified support network
Knowledge of community resources and ability to engage community partners, patients and families in coordinated care planning for community-dwelling frail seniors
Superior interpersonal and relationship building skills
Exceptional computer skills
Exceptional conflict management skills
Demonstrated ability to use best practice and research
Preferred Qualifications Proficiency in French would be an asset.
Experience with Cerner and Clinical Connect
Knowledge of registration and scheduling
CGN(C) CAN Gerontological Certified
