Covenant House is seeking a Follow Up Worker to support youth transitioning out of homelessness. This full-time position is pivotal in conducting housing assessments, offering life skills training, and navigating support systems. You will work closely with clients to ensure they have the resources to thrive independently while documenting progress to assist in evaluations.

Key Responsibilities:
• Conduct housing assessments and compile detailed histories
• Develop actionable and realistic housing plans with clients
• Assist clients in navigating social services and support systems




• Provide tailored life skills training on independent living
• Liaise with housing providers for rent negotiations

Requirements:
• College degree in child and youth work or equivalent
• Familiarity with housing navigation in Toronto
• Experience working with at-risk youth in community settings
• Strong interpersonal and communication skills
• Commitment to anti-oppressive and anti-racism practices
