Drive quality geriatric care at Brant Community Healthcare System as a Nurse in the Nurse-Led Outreach Team. This role involves mobile assessments and mentorship within Long Term Care Homes.

As a key member of BCHS, you will enhance the quality of care for the elderly, addressing acute health changes and fostering educational growth among staff. Key Responsibilities:
• Facilitate smooth transitions to LTC following ED visits
• Conduct comprehensive assessments and care monitoring
• Provide education tailored for LTC staff and caregivers
• Champion patient-centered nursing with expert consultations
• Develop programs to curb acute admissions from LTC facilities

Requirements:
• Bachelor’s Degree in Nursing
• Registered Nurse with the College of Nurses of Ontario
• At least 3 years in geriatrics and acute care
• Updated BCLS/CPR certification
• Geriatric Certificate Program is mandatory
