Join Bruyère Health as a Geriatric Assessor to provide in-home assessments for elderly clients. This full-time role focuses on optimizing health and safety through evaluations and interventions.
As part of the Geriatric Assessment Outreach Team, you will report to the Manager of Outpatient Programs. Your primary responsibility will be conducting comprehensive geriatric assessments to address clients’ biomedical and psychosocial needs. You will collaborate with caregivers and community resources to enhance functional independence among seniors in their communities.
Key Responsibilities:
• Conduct in-home geriatric screenings and assessments
• Analyze clinical data for program evaluation
• Collaborate with clients and their support systems
• Provide actionable health recommendations
• Document assessment findings in a transparent format
Requirements:
• Bachelor’s in Nursing or Master’s in Occupational Therapy/Physiotherapy
• Minimum five years clinical geriatric experience
• Valid driver’s license and vehicle access
• Fluency in French and English
• Registration with relevant Ontario Colleges
