This permanent, full-time position primarily focuses on case management under the guidance of the Adult Outpatient Mental Health Program Manager. With at least three years' experience in community settings, successful applicants will possess robust assessment and coordination skills relevant to clients' unique needs. Learn and implement the ACTT model effectively.

Key Responsibilities:
• Assess clients and develop tailored treatment plans
• Coordinate and provide support services through an interdisciplinary approach
• Address substance use issues with sensitivity and expertise




• Handle crisis situations and manage suicide risks
• Maintain accurate clinical documentation for clients

Requirements:
• Bachelor’s degree in Social Work; registered and in good standing
• Three years of experience in community mental health
• Experience working with individuals with substance issues
• Must possess reliable transportation; valid Ontario Driver’s License
• Exceptional communication and organizational skills
