AgeCare in Toronto partners with Generations Toronto to recruit a Nurse Practitioner for the long-term care home. You will work collaboratively with the MD, DOC, and interdisciplinary team to deliver resident-centered care aligned with competency and standards.
The role involves assessing and managing acute and chronic conditions, prescribing treatments, supporting residents and families, and leading clinical initiatives to improve quality of care across the LTC team.
