Enhance community health at Durham Community Health Centre as a Nurse Practitioner focused on primary care and mental health. This temporary full time role supports accessible health solutions in Oshawa and across Durham Region.
As a Nurse Practitioner, you will provide comprehensive, client-centered care within the West Durham Interprofessional Primary Care Team. Focused on clients experiencing mental health symptoms, your responsibilities include assessments, diagnostics, and treatment planning while supporting clients from marginalized backgrounds. Your leadership and advocacy will further DCHC’s mission to improve health equity and serve vulnerable populations.
Key Responsibilities:
• Provide primary care services for mild to moderate mental health issues
• Conduct clinical assessments and therapeutic interventions
• Collaborate with an inter-professional care model
• Manage client treatment regimes and follow-ups
• Maintain accurate medical records using EMR system
Requirements:
• Master's degree in nursing; registered NP in Ontario
• Minimum three years of primary care NP experience
• Excellent communication and critical thinking skills
• Valid Ontario Driver's license and vaccination against COVID-19
• Flexibility to work across multiple DCHC locations
