Position # : V43430 Expected Start Date: September 21, 2026 Union: HSAS Facility: Wascana Rehabilitation Centre City/Town: Regina Department: LTC Nursing Unit - Convalescent Care Unit Type: Full-time temporary Expected Up to Date: October 08, 2027 FTE: 1 Shift Information: Days Number of Hours per Rotation: 8 hours shift Relief: No Float: No Hours of work: Standard hours Salary or Pay Band: Pay Band Assessor Coordinator - Degree $36.721 to $44.902 Travel Required: Yes Job Description: Under the direction of the Manager, and as part of the interdisciplinary Convalescent Care Team, the Assessor Coordinator is responsible for the full scope of case management services for clients in convalescent care beds.
The core functions of case management practice include referral/intake, engagement, assessment, care planning, monitoring, evaluation, and termination/discharge. This is done through collaboration with the family, care givers, and other members of the multi-disciplinary team.
The Assessor Coordinator liaises with other services in the Regina area, in an endeavor to provide the right service at the right time and in the right place. This position reports directly to the Manager for daily clinical supervision. The Assessor Coordinator works within Convalescent Care.
This position collaborates with acute care, rehabilitation, home care, long term care, mental health and an array of government and non-government agencies to assess clients’ needs and coordinate services to meet those identified needs. Case management is client-centered, strength-based, adaptable, cost-conscious and quality driven.
One of the key functions of case management is to advocate for and coordinate resources based on assessed need. Human Resources Exemption:
No Education Recognized degree in nursing , or a social work degree. Demonstrates effective verbal and written communication skills. Knowledge of theories, concepts, intervention strategies for case mgmt practice Advocacy, negotiation, crisis management and conflict resolution Knowledge specific to populations being served, example: Gerontology & Aging Self-reflective practice: adhere to profess.

Community resources internal and external to the Regina area Demonstrated ability to practice core functions of case management Other Information Registered/eligible with the Saskatchewan Association of Social Workers (SASW) This is a multisite position, home base at Wascana Rehabilitation Centre Requires a valid driver’s license.
May require access to a vehicle for work. Registered/eligible with the College of Registered Nurses of Saskatchewan (CRNS)
