Foothills PCN is seeking a family physician for locum coverage within the Coordinated Attachment program. This prospect may transition to an ongoing position within a caring and collaborative team environment. You will address the needs of patients with chronic health issues, mental health concerns, and social factors affecting health. Key Responsibilities:

- Practice family medicine in a team-oriented setting
- Care for patients with complex health backgrounds
- Collaborate with social workers and healthcare providers
- Engage in effective referral processes with community partners
- Deliver comprehensive assessments and treatments
